Actin' Up Youth Theater Workshop
Combined Policy, Waiver, & Release Agreement
Please read the following sections carefully. By signing at the bottom, you acknowledge and agree to all terms outlined across these sections for your minor child/children participating in the Actin' Up Youth Theater Workshop hosted by Cutting Edge Center of the Arts Inc.
1. Facility & Equipment Inspection
The parent(s) or legal guardian(s) will instruct the minor participant that prior to participating in any performing arts activity or event, they should inspect the facilities and equipment to be used. If the participant believes anything is unsafe, they should immediately advise program officials and refuse to participate. If at any time I or my child feel anything is unsafe, we will take all precautions to avoid the unsafe area and refuse further participation.
2. Assumption of Risk
I/We fully understand and acknowledge that:
There are risks and dangers associated with participation in dance and theater events/activities, which could result in bodily injury, partial or total disability, paralysis, or death.
Severe social and economic losses or damages could result from these risks.
These risks and dangers may be caused by the action, inaction, or negligence of the participant, or the action, inaction, or negligence of others, including the Releasees named below.
I/We knowingly accept and assume all such risks and responsibilities for any losses, injuries, or damages following such injury, disability, paralysis, or death, however caused.
3. Release, Waiver, & Indemnity Agreement
I/We hereby release, waive, discharge, and covenant not to sue Cutting Edge Center of the Arts Inc., including its owners, directors, officers, managers, promoters, instructors, agents, employees, consultants, and hosting studio premises (collectively referred to as "Releasees"), from all liability to the undersigned, my/our personal representatives, assigns, executors, heirs, and next of kin for any and all claims, demands, losses, or damages on account of any injury—including death or property damage—arising out of or relating to the event(s), caused or alleged to be caused in whole or in part by the negligence of the Releasees or otherwise.
I/We acknowledge that these activities involve inherent risks, and that injuries received may be compounded or increased by negligent rescue operations or procedures of the Releasees.
This agreement is intended to be as broad and inclusive as permitted by law. If any portion is held invalid, the remainder shall continue in full legal force and effect. On behalf of the participant and individually, if despite this release a claim is made against any Releasee, the parent(s)/guardian(s) will reimburse the Releasee for any payments made on the participant's behalf and hold them harmless.
4. Media & Photo Release
I understand that photos, audio, and videos are periodically taken during rehearsals and performances. I consent to having photographs, audio, or video recordings of my child used by Cutting Edge Center of the Arts Inc. (its assigns or successors) for publicity, marketing, website, and publication purposes. All media remains the sole property of Cutting Edge Center of the Arts Inc., with full rights to duplicate and distribute free and clear of any claims.
5. Emergency Medical Consent
I hereby give consent for my child to be treated by emergency medical personnel, a physician, or a surgeon in the event of sudden illness or injury during participation with Actin' Up Youth Theater Workshop at Cutting Edge Center of the Arts Inc. I understand that neither the organizer nor the venue provides medical insurance, and all costs for treatment will be my responsibility.
6. Workshop Rules & Operational Policies
Mandatory Parent Meeting: Parents of all cast members must attend the initial orientation meeting.
Parent Participation & Volunteer Commitment: One parent per family is required to fulfill a volunteer position selected on the registration sheet and assist with "strike" (set breakdown, cleanup, and loading) following the final performance.
Communication: Official updates are communicated via Facebook. Parents agree to regularly check the designated Facebook page/group and provide an accurate address upon registration.
Costumes: Cast members provide their own basic costume items or pay a costume rental fee as specified by production leadership.
Absences & Tech Week: Attendance is critical. Cast members who miss rehearsals may be cut from specific numbers, as missed material cannot be re-taught. Conflicts must be submitted with registration. Attendance during Tech Week (the week of the performance) is mandatory—absences during Tech Week are not permitted and will result in consequences for the participant.
Behavior & Dismissal: Cast members must be capable of focusing for up to 90 minutes at a time. Disruptive students will be asked to sit out. Inappropriate behavior by a student or parent may result in immediate dismissal from the program without a refund.
